The crash of Air India Flight 171 on June 12, 2025, has marked a tragic event in aviation history. The Boeing 787-8 Dreamliner crashed shortly after takeoff, resulting in the loss of at least 260 lives, including passengers and crew onboard and others on the ground. This incident underscores the critical importance of safety in air travel.

On June 12, Flight AI171 crashed into residential quarters near BJ Medical College in Ahmedabad just 30 seconds after takeoff. On June 13, India's Aircraft Accident Investigation Bureau (AAIB) began a formal investigation and formed a multi-disciplinary committee. Between June 14 and July, black boxes were recovered and preliminary analysis commenced. By July 14, AAIB released a preliminary report indicating that both engine fuel control switches were abruptly moved from “RUN” to “CUTOFF,” cutting fuel supply and causing engines to lose thrust.

Air India initiated a "Safety Pause," reducing services while the Directorate General of Civil Aviation (DGCA) ordered enhanced inspections of all Boeing 787 aircraft in India.

This crash is noted as the first fatal accident involving a Boeing 787 resulting in hull loss. The aircraft involved was nearly twelve years old and known for its fuel efficiency and advanced systems. Its engines by General Electric GEnx are now under scrutiny following this incident.

Cockpit voice recordings revealed confusion among pilots during the flight's final moments: “Why did you cut off the fuel?” one pilot asked; “I did not do so,” responded another. A MAYDAY distress call was issued seconds before impact.

The AAIB continues its investigation into whether the fuel switch movement was mechanical, electrical, or human error with global aviation authorities closely monitoring due to implications for Boeing’s wide-body aircraft.
